Sun Health is looking for a big-hearted, invested, and thorough Wellness Coordinator to join our vibrant team at Administration Offices in Surprise, AZ.

Statement of Purpose: Upon referral, assesses health and psychological needs and coordinate care services for Independent Living residents. Develops individual wellness plans/interventions for residents and oversees implementation of plans. Establishes linkages with agencies and service providers and monitors the services being administered to residents. Provides education and support to residents as needed. Maintains ongoing coordination of care with Resident Services, and other team members as needed.
Compensation: Hourly and non-exempt from overtime pay. This is a non-management position.

Major Tasks, Duties, and Responsibilities:
- Coordinates services for residents, including psychological assessments, crisis intervention, and education.
- Attends Healthcare committee (HCC) meetings, providing information and recommendations as needed. Keep RSD and others informed of any new referrals, changes in situation.
- Develops individual wellness plans/interventions for residents and oversees plan implementation.
- Reviews medical decisions related to their health care and assists in coordinating identified needs.
- Performs wellness assessments and goals while visiting with residents in their home and educates residents about available services at Sun Health.
- Recommend and coordinate community resources and interventions that maximize independence and promote safety at home.
- Provides advocacy and acts as a liaison between residents and their family and follows up with member or family concerns as needed.
- Follows up with residents during and after hospital or health center admissions. Assists with follow-up coordination of services.
- Educates and assists with the psychological support during transitions to other levels of care.
- Provides medical advocacy, ensuring medical providers and services are coordinated to the maximum benefit of the resident.
- Identifies the need and coordinates support for residents as appropriate.
- Ensures an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment.
- Employs an approach of contact and communication that demonstrates a high level of customer service.
- Ensures the confidentiality of all resident-related information.
- Attends all mandatory in-service training programs and required meetings and takes an active role in performance improvement studies, focus groups, etc.
- All other duties as assigned by the VP of Operations.

Job Type: Full-time
Pay: $35.00 - $38.64 per hour

Schedule:
- 8 hour shift
Experience:
- assisting adults with care coordination wellness programming: 1 year (Preferred)
- Level 1 Fingerprint Clearance Card: 1 year (Preferred)
License/Certification:
- Licensed Practical Nurse License (Preferred)
- RN License preferred (Preferred)
