Weekend On Call RN

For over 35 years, The Hospice of Baton Rouge has been meeting the end-of-life care needs of individuals throughout our 7 parish area and is the oldest continuously operating non-profit hospice provider in Baton Rouge.

The Hospice of Baton Rouge is seeking a Full-time On Call RN to work every weekend and a PRN Weekend On Call RN to work every other weekend. Weekend On Call is from Friday at 4:30 pm to Monday at 8:00 am.

Qualifications:

· Current Louisiana RN Licensure

· Current CPR Certification

Responsibilities:

· Assumes responsibility for weekend on-call duty as scheduled and consistent with Hospice of Baton Rouge policies and procedures.

· Triages phones calls from answering service and make visits as per Hospice of Baton Rouge policies and procedures.

· If more than one call from family is received, a visit is made.

· If a patient falls, offer a visit. If injury is reported, a visit is required.

· If instructions are made over the phone, a follow up phone call is made.

· Current schedule is every weekend.

· Attends Monday IDT Meeting as directed by DON.

· Communicates any concern or issues from weekend with DON and Case Manager.

· Documents calls and/or visits in clinical notes in My Unity within 24 hours.

· Participates in THBR QAPI program.

· Provides updates to case managers as required.

· Participates in agency Performance Improvement activities.

· Understand that scope of job may change based on patient census and needs of agency.

· Attends mandatory staff in-services as requested.

Job Types: Full-time, PRN

Pay: $31.00 - $35.00 per hour

• Nursing: 2 years (Required)
