Agape is a locally owned and operated hospice that has been serving the front range for the last 20 years. We offer safe staffing ratios and work-life balance because we understand that our greatest asset is the people providing care to the patients entrusted to Agape.

The Weekend On-Call Nurse works as part of well staffed on-call team. All incoming calls are triaged by a phone support RN before going to the On-Call nurse. On-call is never the primary phone support, but are utilized for visit needs.

The On-Call nurse responds to patient and family needs Saturday and Sunday. Competent assessment and communciation skills are required to address changes of conditon and provide support and comfort to our hospice patients. Patients are located across the Denver metro area in private homes, assisted living communites, and skilled nursing facilities. The ability to adapt to multiple care enviornments and provide excellent care is a must. You will work with on-call providers and pharmacy.

As part of The Agape Team, you would use your clinical skills to enhance the quality of life for patients by:

• Communicating with patients, family, attending providers, other members of the IDT and healthcare providers as needed to coordinate the best care.
• Completing patient visits as determined by schedule and patient needs.
• Documenting patient visits/calls in electronic health records system.

Required:

• Associates degree in nursing. BA or BS degree preferred.
• Current license in good standing to practice as a registered nurse in the state of Colorado.
• Valid Colorado driver's license, proof of auto insurance and reliable transportation.
