Summary:
Serves as case manager to support, teach, advocate, and empower clients with mental health, medical and
Substance Use needs. This includes developing person-centered recovery goals to live, learn, socialize, work
and maintain health and wellness in the community.

Essential Responsibilities and Target Outcomes:
Provides advocacy, support and guidance with contacting external organizations (Social Security, DSS,

DCF, DMHAS, etc).
 Educates clients to utilize emergency and community resources to help meet basic needs. 
Works with clients to teach life skills necessary to live independently in the community. This includes

assisting with finances and entitlements, supporting the development of activities of daily living,
nutrition, housing maintenance and repair, and prompting to encourage task completion.

 Collaborates and integrates with other CMHA teams to ensure all services provided to client are
communicated with clients whole treatment team

 Assures timely documentation is maintained in accordance with CMHA policies and procedures,
including but not limited to progress notes, treatment plans, authorizations, program orientation, and
consent to treat, activity logs, and monthly targeted case management notes. Maintains appropriate
caseload as determined by CMHA and funder

 Performs other related duties as assigned.

Principal Customers:
• Clients: helping meet their full potential through working towards identified goals.
  • Clients’ families/caregivers/guardians: supports the involvement of natural supports when indicated.
  • CMHA programs/services: collaborating with integrated teams as needed to ensure quality of care.
  • Supervisor: provides support and guidance on all aspects of care.

Critical Work Relationships:
CMHA departments, teams, and staff.
 External providers such as: DMHAS, DSS, DDS, DCF, Landlords, Visiting Nurses, Outside Clinical

and other entities as needed.

Minimum Requirements:
Education: B.S. Degree in a mental health related field preferred or equivalent experience
Credentials and
Licenses:

Must have valid driver’s license, a vehicle,
Will become certified in MANDT, First Aid, and CPR.
Participate in trainings on an annual basis, as assigned.

Experience  Successful experience working in Case Management or related field with a solid
understanding of Mental Health conditions, preferably a minimum of one year.

 Beginner to intermediate experience with Microsoft Office products
including Word and Outlook. 

Experience in or willingness to learn how to use an Electronic Medical
Record (EMR) system.
