Job Summary: Work as part of CFP's Family Networks team at a Department of Children and Families Area Office (Openings in both the Hyde Park and Harbor offices) to facilitate service delivery to your assigned caseloads.

Treatment/Service Coordinator Responsibilities:

• Casemangement duties for assigned caseload
• Coordinating and facilitating meetings for all families receiving home-based services and/or placement services
• Develop treatment plans based on family meetings
• Attend meetings as assigned, which may include traveling around the state
• Complete all necessary paperwork efficiently and in a timely manner
• Proficient in report writing and computer skills
• Work collaboratively to ensure quality service provision is provided

Service Coordinator Qualifications:

• Bachelor’s degree in Social Work or related field
• Work experience with DCF involved families, provider community and/or related service provider
• Knowledge of DCF practices and policies preferred
• Must have valid driver’s license and transportation.

Job Type: Full-time

Pay: $40,000.00 per year
