THE PROGRAM
This Accessible Depression and Anxiety Peripartum Treatment (ADAPT) introduces a partnership between mental health services and public health nurses to provide timely, convenient and holistic mental health treatment to individuals and family members who screen positive for postpartum anxiety and/or depression. This program will utilize trauma-informed, evidence-informed practices to provide in home therapeutic and peer support services to in a setting most convenient for the client. ADAPT staff will be co-located and embedded within the Nurse Family Partnership (NFP) and Maternal Child Health (MCH) program sites in order to establish collaborative relationships, participate in regular case conferences, and be a part of a multidisciplinary team with the referring sources. The ADAPT program will consist of Level One treatment that are higher-intensity services designed to stabilize symptoms of postpartum anxiety and/or depression. Treatment in this level include community based therapy and access to Peer Partners if needed. Level Two services are less intense and designed to increase linkage to existing community resources and supports. Peer partners role in the ADAPT program is to provide education, advocacy, and peer-support services. Peer supports will also establish supportive relationships with parents, particularly those at risk of not engaging in therapy when indicated.
THE POSITION
The Peer Partner provides client engagement, linkage and consultation as well as direct services to a caseload of clients.
· The primary focus areas of direct service include case management, linkage and resource support.
· The Peer Partner maintains client charts, develops service plans, completes assessments and program outcomes, and writes progress reports for the Level 2 clients.
· The Peer Partner collaborates with programs and community agencies such as emergency mental health providers, faith-based organizations, and support groups and utilizes them as part of the multi-disciplinary team.
· Coordinates intake activities and discharge planning.
· Provides rehabilitation and skill building services on topics such as decision making, emotional regulation, etc.
REQUIREMENTS
- Must have a high school diploma or GED.
- Experience with working with clientele similar to those in the assigned Program.
- Must have dependable transportation and be willing to travel to client homes and community locations.
- Bi-lingual preferred
- Must pass pre-employment background check, TB test, drug screening, and COVID vaccine requirements.
HOURS
Monday – Friday
30 - 40 hours per week
$15.50 - $20.00, depending on experience
