Essential Job Functions

• Conduct initial and follow up assessments for assigned clients
• Provide individual orientation for each client
• Deliver regular individual therapy; listen to client concerns/issues and provide feedback when appropriate
• Utilize evidence-based treatment approaches, methods, and interventions to deliver a highly personalized treatment experience
• Closely monitor client progress; prepare and adapt treatment plans based on changing needs and goals of clients
• Complete documentation regarding client care and response to treatment in a timely, clear, and concise manner
• Provide case management and crisis intervention as appropriate, coordinate care with multi-disciplinary team
• Communicate and provide services consistent with Federal Confidentiality and Health Information Portability and Privacy Act
• Travel to client residences to deliver treatment
• Other duties as assigned

Qualifications

• Advanced communication, interpersonal, and conflict resolution skills
• Ability to interact effectively with clients (and other supports as appropriate) to provide support at different phases of recovery
• Ability to think creatively, flexibly, analytically, and critically in quickly changing situations
• Bachelor’s degree required
• Master’s degree, PhD, or PsyD in social work, counseling, or related field preferred
• Minimum 2 years of previous experience in counseling, social work, or related field
• Current Illinois certification as a substance abuse counselor (CADC) and/or licensure (LPC, LCPC, LSW, LCSW)
• Valid driver’s license with clear driving record; ability to travel within the Chicagoland area for home visits
