What You'll Do
Summary:
The Stroke Coordinator promotes optimal patient care through clinical activities, professional and public education, research and quality assurance activities. The Stroke Coordinator works under the supervision of the Manager Comprehensive Stroke Center to achieve improvements in patient outcomes, operational performance and reduction in variability of stroke care provided. This is a multifaceted role, incorporating expertise in clinical practice, data analysis, research, and performance improvement methodologies in order to achieve measurable improvements in the 5 pillars, with emphasis on quality and successful Stroke Certification surveys. The coordinator will help develop, implement, and sustain the Comprehensive Stroke Program for New Hanover Regional Medical Center. The coordinator monitors the day-to-day clinical activities of the Stroke Program and serves as primary contact with clinical areas throughout New Hanover Regional Medical Center and the Coastal region. This person is responsible for the organization and presentation of concurrent data, response process operations, and dissemination of information to the clinical teams. The Stroke Coordinator acts as an effective leader, resource, champion for change, and role model for quality stroke care.
Responsibilities:
1.Maintain in-depth knowledge of AHA/ASA Stroke Guidelines.
2.Respond to Code Stroke, Code ELVO and Code Neuro when possible to help facilitate the reperfusion process and monitor standard work.
3.Conducts concurrent reviews on each Stroke, ELVO, and Neuro case. Collects key data elements and provide feedback to providers within 48 hours. Follows through process improvements on outliers through resolution and loop closure. Documents accurately and thoroughly.
4.Coordinates the aggregation and presentation of concurrent data and performance improvement information to the Comprehensive Stroke Center.
5.Facilitates and participates in or meetings and or teams related to stroke performance improvement. Organizes and prepares stroke debriefings with appropriate attention to details as needed.
6.Is proficient with Lean processes (attends Lean Bootcamp).
7.Assists with stroke education at NHRMC and throughout the region in early identification and treatment of Stroke patients.
8.Provide assistance to the Manager of the Comprehensive Stroke Center in implementing improvement processes.
9.Participates in daily multidisciplinary rounds on stroke patients.
10.Serves as a resource and consultant on issues of standards of care for stroke patients.
11.Assist and participate on the Stroke Teams.
12.Participates in all preparatory and maintenance activities necessary to facilitate successful Stroke Certification surveys.
13.Maintains and adjusts schedule to enhance team performance.
Position Requirements
Credentials: Essential:
- Registered Nurse
Experience: Three to five years of clinical practice, preferred experience in emergency medicine or neurology care. Combined clinical experience including research, data management or program management will be considered. Advanced degree may be considered in lieu of clinical experience.
Demonstrates standards of performance (ownership, teamwork, communication, compassion) that support patient satisfaction and principles of service excellence.
Performs other duties as assigned.
Individual will possess commensurate combination of education, experience and qualifications.
