The Housing Case Navigator is a full-time position that supports the reStart SSVF mission to end Veteran homelessness for all individuals, families, and communities by assisting program participants in locating and securing permanent housing or alternative housing. Working with program participants, landlords, and property management companies, the Housing Case Navigator connects homeless veterans with a full-range of housing opportunities.


The SSVF Housing Case Navigator will be responsible for assisting participants in obtaining permanent supportive housing (PSH) within 30 days and ensuring the housing options meet SSVF requirements. The Housing Case Navigator will customize services to each individual family’s need and will prioritize services that promote appropriate housing stability. The Housing Case Navigator will collaborate with local private landlords and apartment complexes to create and monitor an ongoing housing database of immediate housing opportunities. The updated housing database will be communicated to the SSVF team weekly. The Housing Case Navigator will at all time maintain a caseload up to 20 clients and engage in 5 new community resource opportunities.



DUTIES AND RESPONSIBILITIES

  • Support SSVF program participants with customized housing to meet client needed
  • Provide targeted customer service and mediation support to properties participating, Private landlords in SSVF Program to promote long-term tenancy and partnership
  • Find creative solutions to prevent tenant evictions and support positive tenant and Landlord resolutions by working with team, tenants, and landlords
  • Collaborate with team members to promote client progress and address challenges with daily effective communication through communicate emails
  • Conduct office, home, and field visits to program Landlords to ensure success
  • Network with other agencies, coalitions, and the local community to support program development
  • Reach out and engage with landlords to develop housing opportunities
  • Maintain complete and accurate documentation of service objectives and outcomes as well as other services in accordance with SSVF guidelines
  • Facilitate communication with all Program Manager by preparing Monthly Housing-related reports
  • Manage an ongoing up-to-date housing database system, to includes rating standards, calibration housing partnership
  • Carry a caseload of 20 clients to ensure stability is maintained; Client caseload subject to change


SPECIFIC SKILLS

  • Knowledge of Missouri Fair Housing and Reasonable Accommodations
  • Knowledge of Kansas Fair Housing and Reasonable Accommodations
  • Ability to work with a variety of landlords and property management companies
  • Ability to maintain and execute confidential information according to HIPAA standards
  • Ability to coordinate multiple projects simultaneously in a high-pressure and time sensitive environment
  • A high level of tolerance and understanding of individuals who require services with urgent multiple case management and health needs
  • Ability to work independently and within team environment and exercise mature judgment
  • Willingness and ability to regularly provide community and in-home based services
  • Strong written and verbal communication skills

QUALIFICATIONS

  • Veteran (preferred).
  • Bachelor’s Degree in a human services related field or a related field and two years of experience.
  • Five years cumulative experience working within social services and populations in crisis including the homeless population and people with disabilities.
  • Education and/or experience with property management and realty functions.

MANDATORY REQUIREMENTS

  • Reliable transportation
  • Successful completion of background and drug screening
  • Ability to work flexible hours which include evenings or weekends
  • Ability to obtain CPR/First Aid certification
  • Driving is an essential function of this position
    • Must have Valid MO or KS Driver's License
    • Must provide proof of insurance coverage
