15-20 hours a week M-F based on need.

JOB SUMMARY: The Incident Investigator oversees, tracks, monitors, and ensures compliance with the TBI and NHTD incident reporting process as prescribed by the Department of Health and All Metro Health Care policies, procedures and standards. The Incident Investigator may complete incident investigations as needed and will review all incident reports prior to submission.

DUTIES AND RESPONSIBILITIES:
  • Tracks the submission of all incident reports at all stages of the reporting process including due dates for follow-up investigations
  • Distributes status reports as appropriate
  • Communicates with the participant and Service Coordinator regarding incident status as appropriate
  • Completes incident reports as needed
  • Completes incident investigations as needed
  • Ensures compliance with DOH mandated time frames for reporting process
  • Participates in Serious Reportable Incident committee
  • Participates in and/or gathers information for Quality Improvement committee meetings.
  • Assists in identifying trends by participant, employee, branch, or agency and notifies supervisors as appropriate.
  • Notifies Supervisors regarding status of incident reports.
  • Communicates with government agencies regarding incident reports
  • Develops and submits quarterly and annual reports
  • Maintains ongoing compliance with waiver regulations as well as to prevent fines, penalties, and unfavorable audits.
  • Attends monthly meetings
EDUCATION:
  • Successful completion of the TBI/NHTD Training Course
  • Be at least 18 years of age
  • Bachelor's Degree in Human Services or a related field (preferred) or;
  • High-school graduate or equivalent
  • Experience and/or training conducting investigations (training may be provided by AMHC)
EXPERIENCE:
  • Two years of health and human services experience
SKILLS REQUIRED:
  • Strong organizational skills.
  • Ability to effectively communicate, both verbally and in writing.
  • Strong problem solving and decision-making skills.
  • Basic typing and computer literacy skills.
  • Computer and database skills including Excel.
