The primary purpose of the Self-Direction Assistance (SDA) is to provide service facilitation to individuals and families in the Home Based Services program. SDA assists the individual (or the individual's family or representative, as appropriate) in arranging for, directing and managing services. It also offers practical skills training to enable individuals and families to independently direct and manage Waiver services. SDA includes providing information to ensure that individuals understand the responsibilities involved with directing their services. The Self-Direction Assistance is responsible for developing and overseeing the services for approximately 45-50 individuals served. This position works collaboratively across agency programs and with external service providers to promote the delivery of quality services to people served. The Self-Direction Assistance supports individuals and families to connect with community resources that promote the facilitation of personal outcome measures. The Self-Direction Assistance is expected to uphold the Mission & Values of the Association.
Description of Responsibilities:
- Meeting with Independent Service Coordination agencies for Discovery and/or Personal Plan
- Works collaboratively with providers of direct services to creative positive behavior strategies and plans as needed.
- Adheres to timelines for the completion and review of Individual Support Plans and Person Centered Plans, and necessary assessments. Ensures that documents contain complete, current, and accurate information.
- Monthly contact with individuals and their families, as well as other community provides supporting the individual, are recorded using the approved agency format.
- Monthly billing summaries are completed based on direct/indirect contact hours and submitted to Family Support Administrator by the designated deadline via RGA Database.
- Completes Service Agreements and Service Authorizations according to DHS standards.
- Ensures that virtual files are complete and meet the expectation of RGA and regulatory entities, including the adherence to confidentiality policy.
- Facilitates connection to resources by arranging Program tours at RGA and access to other providers within the community as necessary.
Qualifications Required:
Per Illinois Department of Human Services, Must have at least one year experience working directly with people with developmental disabilities and Bachelor's degree in a Human Services field.
Other Requirements:
Excellent verbal and written skills are essential.
Must have Clear State Police History and Clear Health Care Worker Registry background checks.
A valid Illinois Driver’s License.
Must be at least 21 years of age.
Must pass a pre-employment drug screen
Schedule: Hybrid 2 days office 3 days work from home

Starting Wages/hourly: $19.23
