Bright Sky Home Health Care, Inc. is currently seeking an energetic and compassionate RN to join our team!

Job Summary

The RN plans, organizes, coordinates, and directs home health care for home health patients starting from admission to discharge. Develops and implements the plan of care with the interdisciplinary team that meets that patient and family needs within their homes and communities.

Responsibilities and Duties

RN completes a comprehensive assessment re-assessment of patient and family to determine home health needs; provides direct patient care, develops and coordinates with interdisciplinary team plan of care; supervises LVNs, and CHHAs; serves as the RN case manager to facilitate coordination of care between home health team and other health care setting; provides ongoing education related to home health care; participates in the Quality Management program.

Qualifications and Skills

An active California RN license; a valid CPR/BLS; a valid California Driver’s License with proof of automobile insurance; ability to use electronic devices such as computer and smart phones; ability to navigate the road system; effective communication skills.

Previous experience in home health is preferred but not required.

Spanish speaking is a plus!

Must be Willing to travel to Perris, Moreno Valley, Nuevo, Homeland, Menifee, San Jacinto, Winchester, and surrounding areas.

RATES:

Per hour: $45-$50

Per Follow-Up Visit: $80

Per Home Health Admission: $140
