Job description

The Stroke Nurse Patient Navigator serves as the point of contact for the coordination of patients with a stroke diagnosis. This person coordinates clinical resources, assesses clinical and supportive care services and plans for community support before and immediately after discharge. This position provides information to the patient and family to prepare them for discharge with a stroke related diagnosis and to help navigate them through the processes: facilitating appointments, equipment and follow-up care. Additionally, this position works closely with Performance Improvement to ensure WJMC maintains compliance with accrediting agencies. This position also provides community education for the various programs and services available at WJMC.
Job Specifications - Knowledge and Skills
Work successfully in a stressful environment, relate well to all customers, and maintain expanding knowledge and use of hospital information systems. Clinical knowledge in neurosciences/spine care with an understanding of diagnosis and the various treatments. Working closely with physicians regarding the flow of services and treatment options for patients.

Job Specifications - Education and Experience
Education:
Graduate of LPN or RN degree from an accredited nursing school.
Experience:
Five (5) years clinical experience in specialty as a Registered Nurse of LPN with demonstrated experience in stroke and/or orthopedics in a hospital setting. Strong background in stroke and/or orthopedics preferred.
License/Certification:
Valid Louisiana license or temporary permit to practice professional nursing. American Heart Association BLS-HCP. Specialty Certified Nurse, preferred.
