Nurses are assigned to a specific a territory that allows less driving and more meaningful interactions with patients. Provide direct skilled nursing care to patients/clients in the home setting. Critical multidisciplinary collaboration in all areas including the physician and patient/client/caregiver to reach the goal established in the plan of care. May function in an indirect patient care support role.

Qualifications

• Associate’s Degree in Nursing Required
• Bachelor’s Degree in Nursing Preferred
• 1 year of relevant experience in direct patient care Required
• Registered Nurse State of MI Required
• MI Driver’s License Required
