Job Description: Full Time

We are looking for a passionate, professional RNs to join our growing team. The ideal candidate will have at least one year of experience in Home Health. They must possess a dedication to healthcare and sincere desire to help our patients.

Key Responsibilities:

Provides effective and efficient patient care under the supervision of a registered nurse and in accordance with the patient’s plan of care, applicable law and regulation, accepted standards of nursing practice, payor requirements and policies and procedures.

• Participates in ongoing interdisciplinary assessment of the patient.
• Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
• Provides services that are ordered by the physician as indicated in the plan of care.
• Counsels and educates the patient and family in meeting healthcare related needs.
• Completes timely and accurate clinical notes including addressing patient’s progress.
• Assists the physician in performing specialized procedures and duties.

Qualifications:

• Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the state(s) in which you are applying to work
• At least 1200 hours of experience as an RN or completion of the Office's preceptor program.
• Three (3) years relevant experience in medical services or clinical environment with knowledge of medical terminology including previous intake experience of at least one year - Home Health experience preferred

Pay: $50.00 - $70.00 per hour
