Preferred Primary Care Physicians is currently recruiting for full-time, salaried position of RN Chronic Care Manager. The position would be fully remote, but the candidate would have to attend scheduled meetings at the PPCP Central Office in Canonsburg, PA.

General Purpose of the RN Chronic Care Manager Position: Manages targeted patient populations to achieve efficient and effective care
delivery according to the Care Management Guidelines approved and provided by Preferred Primary Care Physicians.

Schedule: Position works Monday - Friday, daylight hours, and additional time as needed to complete work.

Location: Position is one of a remote nature, but the RN Chronic Care Manager will be responsible for attending meeting at the PPCP Central Office in Canonsburg, PA.

RN Chronic Care Manager Essential Duties and Responsibilities:

• Understands and implements principles of Chronic Care Management per guidelines developed and

• Identify, along with the Primary Care Provider, patients eligible for Chronic Care Management services
• Educate eligible patients on the Chronic Care Management program. Enroll appropriate patients in the

• Establish care plan for enrolled patients. Provide care plan to patient and members of care team.
• Working with the Primary Care Physician, provide the non face-to-face services required by the patient.
• Coordinate appropriate care among all providers.
• Ensures continuity of care through defined, evidence -based methods, including, but not limited to,

• Promotes and facilitates improved clinical outcomes and patient satisfaction, as well as efficient use of

• Document services provided in the patient's chart.
• Utilize ENLI Care Manager to Track patients, care needed and provided
• Coordinate and Facilitate IDC meetings with appropriate office staff and the patients care team.
• Monitor program effectiveness at the point of care (e.g. physician offices) and provide feedback and

Knowledge/Skills:

Education: Current Registered Nursing Credentials

Experience/On the Job Training:

Minimum 3 years in nursing with Chronic Care Management experience preferred.
Other Clinical experience will be assessed and accepted.

Credentials:

Current licensure to practice nursing in the state of Pennsylvania
Current Drivers License
