The RN is a qualified professional who directs, supervises, evaluates, and provides nursing services to patients accepted for care by the RVNAhealth as prescribed by the attending physician. This role will also assist in leading to quality referral generation to the agency’s various service lines within the ALF’s. Activities will include relationship building with the referral sources and families within the assigned Assistive Living Faciality (ALF)

Responsibilities:

· Comprehensive evaluation of patient's level of function and participation in development of the total patient care plan in conjunction with therapist (if applicable), patient, family, and physician.

· Perform Oasis assessment visits accurately and at appropriate time points.

· Identification of patient and family needs for other home health services, referral for same when needed.

· Participation in case conferences/ team huddles/ facility meetings. Ensure proper documentation.

· Instruction of patient, family and other agency health care personnel in the patient's treatment regime when indicated.

· Supervision of Home Health Aides as needed by the agency and when such personnel are participating in the patient's care regime.

· Provide proper Supervision and oversite of CNA’s and ensuring proper communication and documentation

· Assists patients to reach a level of function to enable them to remain safely in their home environment while reaching national standard for outcomes and maintain fiscal responsibility.

· Communicate properly and timely with field and office staff as needed.

· Established, and regularly communicating with the coordinator regarding caseload status.

· Being able to Identify, Report to MD and Obtain verbal and written orders for any change of condition.

Qualifications:

· Work within the Business Development Team to enhance awareness of RVNAhealth individual service lines and community programs among all strategic referral sources.

· Provide in-service presentations to appropriate staff to increase customer service and maximize the agency’s team approach.

· Work closely with Building Account manager to ensure smooth transition onto RVNAhealth service as patients discharge home with a comprehensive plan of care.

· Work closely with ALF staff to ensure referrals and transition of care.

· Ensure good commincation between the patient, their families and assist in coordination of care within the ALF.

· Continue to assess for, educate and facilitate right care right time initiative for all patients.

· Make proper referrals across the care continuum as needed
