We are a Central Pennsylvania based Non-profit Hospice & Post-Acute Healthcare Organization specializing in home-centered care and we are seeking a talented Full Time RN Admissions Intake Nurse who is responsible for the overall coordination of incoming referrals and initial visit scheduling under the direct supervision of the Director of Admissions. The RN Admissions Intake Nurse is a resource person for referral sources, and works closely with other members of the interdisciplinary team (IDT) to ensure adequate coordination and a smooth transition occurs to hospice services.

Schedule: 40 hours/Monday through Friday – 8:30am to 5:00pm with occasional weekend coverage

Qualifications:

  • RN with current Pennsylvania license
  • Minimum of one year clinical nursing experience in hospice or home health
  • Excellent communication skills
  • Ability to track and complete multiple tasks at one time
  • Ability to work independently; self-motivated
  • Actively supports and promotes HCP philosophy of care and Mission
  • ICD-10 Experience a plus
  • Proficiency with Microsoft software (Outlook, Work, Excel)
  • Willingness to learn new Computer systems as needed
  • Ability to sit or stand for prolonged periods of time
  • Ability to spend significant time handling telephone calls
  • Ability to create and maintain excellent customer relationships telephonically
  • RN, with current PA license

Responsibilities:

  • Serves as the interface among HCP and referral sources to obtain and exchange referral information.
  • Obtains and reviews initial referral clinical information to make a determination about patient eligibility to elect a hospice benefit.
  • Collaborates with the Medical Director, Hospice Physician, and attending physician as needed to establish eligibility to elect a hospice benefit and establish an initial plan of care.
  • Coordinates and schedules assessment visits for the Admissions staff.
  • Creates the initial referral record and any related orders in the electronic health record.
  • Assists with coordinating pharmacy orders, medical supplies, infusion therapy and any durable medical equipment needed for new referrals and existing patients as requested by care delivery staff.
  • In collaboration with accounting staff, verifies patient benefit information and obtains initial prior authorizations as necessary.
  • Verifies physician license and related credentialing information on referrals by physicians new to the system.
  • Effectively educates prospective patients, families, physicians and hospitals regarding the hospice benefit, organization services and patient eligibility.
  • Prepares reports as requested.
  • Serves as a liaison between clinical and marketing staff.
  • Identifies and reports areas for improvement and concerns for the safety of patients, family members and staff.
  • Consistently conducts daily job responsibilities in a manner that maintains excellent customer service for patients, families, referral sources and staff.
  • Performs other duties as assigned by the Director of Admissions.
