Provide clinical expertise for Value-Based Care Kidney Disease Program within RN scope of practice. Coordinate with Medical Directors, Clinical Operations, Training, and Product Development to identify and provide ongoing training and in-services to Kidney Care Team staff. Maintain and update evidence-based clinical guidelines and maintain standards of practice. Conduct call evaluations, one-to-one training sessions and chart audits. Report to Manager, Value-Based Care, Education.

ESSENTIAL DUTIES AND RESPONSIBILITIESinclude the following. Other duties and tasks may be assigned.

• Provide clinical expertise for Value-Based Care, Kidney Disease Program within RN scope of practice.
• Coordinate with Medical Directors, Clinical Operations, and Product Development to identify and provide ongoing training and in-services to Kidney Care Team staff.
• Provide clinical and non-clinical support and information to counseling and outreach staff. May include case studies, presentations at staff meetings, or individual meetings with staff.
• Provide input on content and parameters of clinical audit tools and assist with educational needs identified through such audits
• Conduct call evaluations, one-to-one training sessions and chart audits.
• Research relevant publications for updates to evidence-based guidelines at least annually
• Maintain and update evidence-based clinical guidelines and maintain standards of practice
• Provide input on changes to product descriptions and program practices
• Share information regarding clinical updates to counseling staff
• Provide systems, communication, and other related training as needed
• Provide training and support to new care team members as needed
• May assist with policy/procedure revisions and dissemination of new and revised policies.
• Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization.
• Participate in team concepts and promote a team effort; perform duties in accordance with company policies and procedures.
• Regular and reliable attendance is required for the job.
