Provide clinical expertise for Value-Based Care Kidney Disease Program within RN scope of practice. Coordinate with Medical Directors, Clinical Operations, Training, and Product Development to identify and provide ongoing training and in-services to Kidney Care Team staff. Maintain and update evidence-based clinical guidelines and maintain standards of practice. Conduct call evaluations, one-to-one training sessions and chart audits. Report to Manager, Value-Based Care, Education.
ESSENTIAL DUTIES AND RESPONSIBILITIESinclude the following. Other duties and tasks may be assigned.
- Provide clinical expertise for Value-Based Care, Kidney Disease Program within RN scope of practice.
- Coordinate with Medical Directors, Clinical Operations, and Product Development to identify and provide ongoing training and in-services to Kidney Care Team staff.
- Provide clinical and non-clinical support and information to counseling and outreach staff. May include case studies, presentations at staff meetings, or individual meetings with staff.
- Provide input on content and parameters of clinical audit tools and assist with educational needs identified through such audits
- Conduct call evaluations, one-to-one training sessions and chart audits.
- Research relevant publications for updates to evidence-based guidelines at least annually
- Maintain and update evidence-based clinical guidelines and maintain standards of practice
- Provide input on changes to product descriptions and program practices
- Share information regarding clinical updates to counseling staff
- Provide systems, communication, and other related training as needed
- Provide training and support to new care team members as needed
- May assist with policy/procedure revisions and dissemination of new and revised policies.
- Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization.
- Participate in team concepts and promote a team effort; perform duties in accordance with company policies and procedures.
- Regular and reliable attendance is required for the job.
QUALIFICATIONS/REQUIREMENTS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
Requirements include:
- Registered Nurse with a Bachelor's degree or higher (i.e. BSN, MSN, etc.). Nursing license must be maintained as current and in good standing.
- Five (5) plus years of experience working with patients with kidney- related health conditions.
- Excellent leadership and coaching skills.
- Demonstrated analytical and problem-solving skills are required. Ability to read, analyze and write reports.
- Proficiency with learning development software a plus.
- Proficient computer skills, including Microsoft Office (Word, Excel, PowerPoint and Outlook); proficiency in all USRC applications required within 90 days of hire.
