Remote RN Care Manager (For Connecticut Residents Only)

• 80% remote
• 20% in the field with members performing assessments (Not a hands on clinical role)

Responsibilities:

• Acts as a clinical coordinator collaborating with members, providers, facilities and a supportive team of multi-disciplinary colleagues to conduct comprehensive assessments designed to identify the member’s medical, behavioral and social determinants of health
• Evaluate member needs
• Develops a person-centered specific care plan to establish specific goals and overcome barriers identified during the assessment.
• Develops, monitors and evaluates the effectiveness of the care plan and modifies as necessary.
• Assists the member by functioning as an advocate by communicating effectively with other members of the healthcare team and by utilizing available resources to promote quality cost effective outcomes and member autonomy.
• Provides education and coordination of outpatient care to Medicaid enrollees identified for Case Management.
• Identifies and refers quality of care, coordination of benefits and third party liability issues as appropriate.

Qualified Candidates will have the following:

• Must be a Registered Nurse with at least 4+ years of experience and an active, unrestricted CT license.
• At least 2 years of home health experience preferred.
• Managed care and case management experience preferred.
• Must reside in Connecticut.

Job Type: Full-time

Pay: $83,000.00 - $86,000.00 per year
