Remote Care Coach (Virtual Medical Assistant)

This will help to drive positive health behavior change and improved patient outcomes.
Role
- Able to care and educate senior citizens to use the remote patient monitoring devices such as Blood Pressure Monitor, Glucose Monitor, etc
- Increase the patient adherence to the program by doing weekly follow-up and providing motivational support on the telephone
- Be responsible for reviewing the readings, keeping patient notes, and informing the Doctor on elevated readings.
- Use appropriate community resources to provide preventive health and disease management education.
- Comfortable with technology, eager to learn, and stay up to date to improve quality of services
Qualification
- High school diploma, GED or equivalent or have 1 or 2 years of post-high school education or a degree from a two-year college.
- Certified Medical Assistant (CMA) (Preferred)
- Experience as Medical Assistant for more than 2-3 years or worked in Home Care Services
- Bilingual - fluent in English and Spanish preferred
- Located in West preferred
Job Types: Full-time, Contract
Pay: $16.00 - $20.00 per hour
