Responsibilities
JOB SUMMARY
Demonstrates the ability to coordinate data collection and data entry for cardiac patients and stroke patients
that meet inclusion criteria for the registry. Assist the Cardiac/Stroke Coordinator and the Director of Emergency
Services in the collection of data related to this patient population. The Cardiac/Stroke Registrar will be expected
to write reports using the software for prevention programs, educational programs and committee reports.

1. Data: Reviews ESD and inpatient records to identify cardiac/stroke admissions, transfers in and out, and deaths. Acquires patient's record post-discharge for review, analysis, data completion, and entry into the registry.

ACCOUNTABILITIES
ESSENTIAL
1. Data: Reviews ESD and inpatient records to identify cardiac/stroke admissions, transfers in and out, and deaths. Acquires patient's record post-discharge for review, analysis, data completion, and entry into the registry.
2. Registry Maintenance: Reviews cardiac/stroke patients for appropriate charge entry. Assumes the maintenance of the cardiac and stroke registry for accuracy and integrity. Downloads on a continuous basis to the required registry.
3. Quality Improvement: Participates in the development of multiple summaries and reports from the registry software to identify records which require review. Assists in the development of the Cardiac/Stroke QI Program by identifying records that require review according to the accepted guidelines.
4. Financial Resources: Clocks in and out per policy. Gets Director permission before working overtime.
5. Personal & Professional Growth: Attends 100% of all mandatory meetings. Attends 80% of staff meetings and/or unit meetings. Maintains training and updates of registry requirements and accreditation.
6. Privacy: Assures privacy of all patient information to protect patient rights and meet HIPPA guidelines. Reports suspected violations immediately.
7. Environment & Patient Safety: Maintains patient and work environment to maximize safe working space. Promptly removes all used and soiled equipment, linen, etc. to appropriate location. BMV scanning rates comply with current policy. Reports all patient safety and hazard concerns immediately.
8. Time Management: Assists Director as requested with tracking time and attendance, overtime analysis, adherence to HR attendance policies and counsels accordingly.
9. Meeting/In-service Attendance: Attends in-services, workshops, and reads current journals to keep abreast of new developments in the medical and nursing fields. Attends 6 unit meetings per year, all mandatory in-services, and completes annual competencies. Attends other meetings as designated by Director.
