POSITION PURPOSE : To conduct RN assessments for clients authorized through the Veterans administration, to improve care for Vets and increase number of Vets receiving care in the four corners area. This is a salary exempt position. This position reports directly to the Care Director.
Pay: $75,000 a year plus Benefits and Car

PRINCIPAL ACCOUNTABILITIES:

  • To oversee or conduct the start of care and ongoing assessments for Veteran clients.
  • To oversee other Contract RN’s who service Veteran Clients in each area we serve.
  • Coordinate with area managers in developing and updating a care plan for each case.
  • Ensure compliance with all requirements of care authorized by the Veterans Administration.
  • Increase the hours of care authorized by the Veterans Administration.
  • Open new markets for Veterans Care.
  • Be efficient in each aspect of your duties.
  • Participate in Marketing efforts to grow Veterans Care.
  • Participate in all company lead meetings, and KPI’s
  • Create, build, and maintain relationships with the area VA offices, and District offices for Veteran’s care.
  • Oversee and conduct HHA training, and other assigned trainings to build the skills of the caregivers serving the clients.

SPECIFIC JOB KNOWLEDGE, SKILLS AND PERSONAL ABILITIES REQUIRED:

  • RN licensing in New Mexico and Colorado
  • Five years of experience in Nursing Assessments and/or Home Health or Geriatric Care.
  • Demonstrate proficiency with Microsoft Office (Word, Excel, PowerPoint and Outlook) applications, scheduling systems and other health care industry related software.
  • Ability to listen and communicate clearly, fluently, diplomatically orally and in writing.
  • Ability to remain flexible, resilient, calm and maintain a sense of humor; and present a well-groomed professional image.
  • Ability to plan, organize, prioritize, delegate and accurately follow through in work activities with time constraints and interruptions to meet deadlines, as well as, work independently with a minimum amount of direction and/or supervision.
  • Ability to generate goodwill for the organization and its management among staff, clients, and referral sources. Demonstrate a strong commitment to client service excellence.
  • Possess and maintain good physical and mental health, including current TB testing.
  • U.S. Citizen or evidence of valid Alien Work Permit.

PHYSICAL/ENVIRONMENTAL DEMANDS:

  • Sitting, standing, bending, reaching, stretching, stooping, walking, climbing stairs and moving intermittently during working hours.
  • Must be able to lift at least 50 lbs.
  • Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met.
  • Must be able to properly operate office equipment.
  • Must be able to maintain verbal and written communication with co-workers, leadership team, supervisors, clients, family members, vendors and all business associates within or outside the agency.
