The Clinical R.N. Nurse Consultant (CNC) is responsible for providing a nursing consultancy services to our Neurorehab facilities. The R.N. Nurse Consultant will provide clinical leadership and specialty related education, training and support to staff within the Programs. The R.N. Nurse Consultant will help to oversee, review and coordinate any medical/physical supports and outcome measures of client care with particular regard to the community- based Neurorehabilitation Clinical programs. This position embeds the concepts of equity, client-centeredness, goals of independence, quality health improvement, and timeliness and efficiency of care. The goals of this position will focus on the need for excellence in medical related services/supports, education, on-site training and organizational support. Must be highly organized and have the willingness and ability to travel to designated program sites within the Southern and Central Maine regions.
Job Summary:
- The Clinical Nurse Consultant provides expert clinical advice to healthcare support for professionals within a defined specialty.
- The Clinical Nurse Consultant develops and facilitates the implementation and evaluation of medical management and support of client care plans for patients with complex health needs.
- Provides a complex-centered consultancy practice within a mixed clinical environment and/or across a series of services (eg. Neurorehabilitation Clinics, Residential Supported Living programs)
- Participates actively in treatment planning process with service recipient and team.
- Provides positive health promotion/preventative health supports and guidance, disease management, in-home medical treatments and rehabilitative care.
- Collaborates closely with the RN [Residential] Educator in the development and delivery of educational resources assisting in identifying and developing specialized educational training resources for care givers (DSP) professionals.
- Act as a liaison between the different components of the service programs to promote continuity of care and exchange of updated information.
- Contribute to formal service and strategic planning processes within the organization.
- Conducts regular and consistent assessments of program clients helping to identify specific needs in relation to the client’s current level of care and changing medical conditions. This includes regular medication reviews and MAR record reviews.
- Conducts on-site training and support to staff in relation to specific client/resident medical issues/needs including use of adaptive equipment, medical support apparatus and medication administration.
- Regularly collaborates with Program Staff, Program Managers, Program Coordinators and Program Directors to help develop and deliver training specific to the clinical specialty.
- Ensures effective delivery and support of quality patient care for consumers with difficult to manage behaviors
- Participation in Quality Improvement activities and the accreditation process.
- Highly developed written and verbal communication skills with strong interpersonal and problem- solving capabilities.
- Demonstrated clinical skills and the ability to provide expert clinical education and training regarding complex issues in a hands-on manner at various program site locations.
- Demonstrated commitment to community integration, independence and consumer participation.
Minimum Qualifications:
- Current State of Maine License as a Registered Nurse
- CRMA Train-the-trainer certified within 6 months of hire
- Current basic level American Red Cross Adult CPR (or higher level) certification or equivalent
- 3-5 years experience in similar training position and/or organization
- Ability to use a PC to include: e-mail, Internet, and Microsoft Office software and effectively use AV equipment
- Demonstrated ability to develop learning objectives & curriculum for training/instruction
- Demonstrated knowledge of adult learning theory and adult learning styles
- Prior experience in presentations to groups up to 50 people
- Reliable transportation with proof of auto insurance
- Valid driver’s license permitting operation of a vehicle in the State of Maine with a satisfactory driving record
- Criminal background check that meets Agency standards
Preferred Qualifications:
- Bachelor’s degree
- Completion of Maine’s CRMA “Train the Trainer” course
- CPR and First Aid licensed Instructor
