WMEC offers a hybrid schedule to all employees after completion of training. Our hybrid schedule allows employees to work up to 2 days remote. Our offices are located in Holyoke, MA.
POSITION SUMMARY:
The CAE, Home Care RN participates as a member of the interdisciplinary HC team in the review, integration, and development of the comprehensive service plan and conducts on site assessments/reassessments/clinical determinations to respective HC and non-HC consumers within the WestMass ElderCare, Inc., service area.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Complies with all current and applicable state, federal, and agency regulations, policies, practices, procedures pertaining to Clinical Assessment & Eligibility (CAE) and the HC programs.
2. Assesses consumers per established standards, including MassHealth and Non-Mass Health recipients/applicants to evaluate clinical status for Community Based and Nursing Facility options, per Commonwealth of MA NF Regulations.
3. Performs initial on-site, comprehensive health assessments to determine personal care, behavioral care and/or home health aide needs, and develops appropriate care plans, while maintaining a reassessment caseload.
4. Ensure Preadmission Screening & Resident Review (PASRR) compliance for any assigned Community or CAE related screenings/assessments.
5. Reviews and completes Pre-Admission NF screening requests from affiliated hospitals/rehabilitation facilities and issues appropriate clinical decisions.
6. Participates in the Interdisciplinary (IDT) Review of HC/CAE Consumers and makes appropriate recommendations
7. Works collaboratively with Care Manager’s as a consultant relating to clinical assessment, as well as review of consumer status for continued appropriateness of the care plan as consumer’s condition changes.
8. Develops and maintains effective relationships with service providers, hospitals and other community organizations in order to be cognizant of potential community resources.
9. Acts to both educate the consumer and caregiver in relation to the care plan, within the scope of practice and role of the ASAP RN, as well as an advocate to the consumer in response to their PC needs and goals.
10. Ensure organized, accurate, and confidential completion of the required computer data, including Comprehensive Data Set (CDS) assessment, all supporting documentation and submission of any applicable MassHealth (MH) Level of Care Clinical Notifications within the required time frames.
11. Assists in the preparation and representation for all appeals in the Fair Hearing Process at the Board of Hearings.
12. Attends all required staff meetings, in-services, relevant outside training programs.
13. Performs and assumes other related duties/special assignments within the scope of the responsibilities of this position as requested by the Nurse Manager or the Director of Home Care.
REQUIRED KNOWLEDGE SKILLS AND ABILITIES:
· Bachelor of Science degree in Nursing or Associates of Science degree in Nursing and at least one year of clinical experience and/or nursing in a community or long term care setting; or a diploma RN and two additional years of community-based nursing experience or long term care direct service experience.
· Experience in community, home health, case management or nursing facility care preferred.
· Knowledge of the aging process, mental health and developmentally challenged populations preferred.
· An understanding and acceptance of the role of the Registered Nurse at WestMass ElderCare which includes: is multifaceted and includes but not limited to the following:
- On-site Assessments to determine need for Personal Care
- Developing Care Plans on all PC Consumers
- Conduct Clinical Assessment & Eligibility (CAE) screenings
- Referral to appropriate agency and/or service
- Monitoring Adherence of Documentation Standards
- Teaching as it applies to the consumer care plan
o Consulting with care mangers regarding the medical assessment and consumer needs
o Working as part of an interdisciplinary team to assist NF residents/consumers in overcoming barriers to discharge by formulating and implementing a care plan that meets the consumers’ needs in the community.
o Non Skilled Nursing Care Only ie., NO Direct Nursing Care/Treatments; NO obtaining Physician’s Orders
