Job Summary/Description:

In this position you will provide quality oversight of assessment and care planning for home health, hospice and home care patients. Provide assistance and training to clinicians to promote full understanding of their responsibilities related to documentation and clinical decision making. Provide ongoing quality oversight of delivery of home health, hospice and home care. Communicate and collaborate with other members of the home health, hospice and home care teams.

Education, Training and Experience:

Minimum of two years experience as a RN

Knowledgeable of applicable local, state and federal regulations.

Current CPR certification ( Basic Life Support)

Must have strong computer skills to include Excel, Word and Power Point

Must have office equipment experience. ( copier, fax, multi line phone system etc.)

Possess good verbal and written communication.

Must be able to meet deadlines, work under limited supervision and be able multi task.

Possess a Valid state drivers license

Show proof of vehicle insurance
