Position Status: Full Time | Monday through Thursday

YOUR ROLE at NWHS:
Diagnose, triage, and provide sample psychotropic medications as well as prescriptions for necessary Psychotropic medications. Monitor medications for an ethnically diverse patient population with medical and behavioral health problems. Provide brief intervention strategies and develop treatment plans for patients. Collaborate and consult with medical providers, mental health staff, and other collaborative partners to ensure effective treatment for patients with behavioral health disorders.
DUTIES:
- Assist primary health care providers in diagnosing and treating behavioral health and psychological issues, through diagnostic consultation, evaluation, and treatment.
- Conduct Psychiatric Evaluations and define the most effective out-patient treatment plan for the client, including referrals for supportive services to effectively support the client’s stabilization.
- Prescribe, follow-up and monitor effectiveness and side-effects of prescribed psychotropic medications with select patients.
- Maintain appropriate medical records documentation on all patients in accordance with organizational policy and agency initiatives including Meaningful Use and Patient Centered Medical Home. Consistently up to date on charting. Meets expectations of above 90% on weekly documentation report and maintains PAQ at less than 75 items at any given period.
QUALIFICATIONS:
- Psychiatric Nurse Practitioner possessing unrestricted license for practice in the state of Oregon.
- Three year's work experience and specialized training in the mental health field.
- Excellent working knowledge of behavioral medicine and treatment for mental health conditions.
- Ability to make quick and accurate clinical assessments of mental and behavioral conditions and develop appropriate management/treatment plans.
- Organized, creative, flexible, open-minded and able to multi-task.
- Able to work effectively as a team, interfacing with patients, primary care providers, mental health staff, as well as administrative and support staff.
