About the Role:

The Project Director will serve as the lead staff person in the design and implementation of the Certified Community Behavioral Health Clinic (CCBHC) model for Community Care Network/ Rutland Mental Health Services. CCBHCs provide a comprehensive range of behavioral health services directly or in partnership with Designated Collaborating Organizations (DCOs) with a focus on coordinated care and the use of evidence-based practices. The process of designing a CCBHC model for CCN/RMHS will be a multi-year effort and will involve significant collaboration across sectors.

As the CCBHC project lead, you will provide overall project leadership and management; plan, direct, and coordinate the work of all necessary personnel across offices, advisory groups, and vendors to accomplish CCBHC objectives; establish work plans, timelines, project milestones, and identify the staff/organizations responsible for each activity; write and present updates and reports on project activities to various audiences; deliver or co-deliver presentations to external stakeholders and committees regarding implementation activities and accomplishments.

Responsibilities:

• Ensure that all CCBHC program development plans and activities are executed in keeping with the funding requirements of SAMHSA and with CCN values, mission, vision, and strategy for the implementation of the CCBHC.
• Serve as the primary contact with SAMHSA for the CCBHC grant.
• Develop appropriate protocols and workflows to ensure the program is set up as intended and proper measurables and deliverables are on target per the Notice of Award (NOA) and SAMSHA guidelines.
• Lead the planning and execution of CCBHC-specific workgroups both internally and externally.
• Provide meaningful feedback and an overview of outcomes throughout the implementation to agency and Designated Community Organization leadership.
• Ensure key components of the CCBHC project are completed according to mandated timeframes.
• Synthesize, analyze and report on data on a variety of levels (client, program, claim, etc.) to Project, Program, Partner, and Agency Administration.

Qualifications:

• Master's Degree in Human Services field (Public Health, Health Administration, Social Work, Psychology, etc.); equivalent experience may be considered.
• Demonstrated project and grant management experience; experience working with integrated care collaborative models preferred.
• Experience in working with the various populations in Rutland County.
• Excellent interpersonal skills.
• Solution-oriented.
• High degree of personal organization, attention to detail, and time management skills
• Proficiency in word processing and other computer functions using Windows, Microsoft Office Suite, and EMR.
• Ability to work with others in a positive, responsible manner.
• Strong writing and organizational skills to receive and convey information efficiently and effectively.
• Ability to maintain confidentiality.
• Flexibility, dependability, and ability to work independently and collaboratively.
• Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
• Successful completion of a criminal history background check after hire.
