Summary:
The Population Health Program Manager is responsible for managing programs related to the shifting of F&MCW from a fee-for-service care & business model to a value-based care & business model. Responsible for achieving strategic objectives through defining, planning and tracking necessary resources as well as providing the direction to various teams to meet program outcome(s). The program manager partners directly with clinician and care teams to improve clinical practice and outcomes and directs quality, finance, analytics and IT resources. They are responsible for sustaining reliable processes and closings gaps identified through participation in quality improvement workgroups, integrated network(s), CMS and other payer programs, etc. The Program Manager applies change management, performance improvement, analytic, technical and business principles to a portfolio of programs that are thoughtfully progressed over time as F&MCW shifts payment models towards value-based care.
EXPERIENCE DESCRIPTION: A minimum five years of experience in a health-related environment in clinical operations, strategy, technology/ innovation, finance or analytics.
EXPERIENCE PREFERRED: Experience in a value-based health care environment like a clinically integrated network / ACO structure or Payer is preferred. Experience with large scale organizational change/improvement and strategy is preferred. EDUCATION DESCRIPTION: Bachelor's degree is required.
EDUCATION PREFERRED: Masters degree in business, health or engineering related field is preferred.
TRAINING PREFERRED: Prior change and/or performance improvement experience using formal methodologies is preferred.
SPECIAL SKILLS DESCRIPTION: 1. Executive Communication - verbal and written 2. Organization of complex materials for distribution among clinical and business leaders 3. Leadership capabilities 4. Proficient level of skill in MS Office suite 5. Ability to collaborate with a variety of stakeholders 6. Project management skills 7. Knowledge of Change Management, PDSA or other performance improvement methods. 8. Must have expertise in contemporary evidence-based clinical best practices, data-driven decision making, and executive communication skills.
LICENSURE PREFERRED: Certification in Change Management and/or project management preferred.
