Job description
Responsible for delivering and facilitating access to comprehensive sexual and behavioral health services for clients at risk for HIV/STI/HCV, including HIV/STI/HCV testing and treatment, PrEP and PEP, navigation and linkage to medical and social services, and risk-reduction education and counseling informed by training in evidence-based interventions. Will conduct program outreach to disseminate information and materials in the community regarding HIV, sexual health, and biomedical interventions to prevent HIV transmission (PEP and PrEP), strategies for risk reduction, and the availability of comprehensive sexual health and HIV services at the Medical Center and to recruit clients to access these services.
Duties and Responsibilities:
* Conduct rapid HIV testing and counseling with clients.
* Conduct community outreach and recruitment targeting individuals at high risk for HIV and STI transmission.
* Assist new and returning clients in setting up and maintaining medical appointments at Wyckoff for sexual health services, including HIV and STI testing, prevention, and treatment
* Facilitate linkage to healthcare and social services for clients and follow-up on client referrals to services to verify linkage, i.e.,: PrEP, PEP, substance use and mental health, benefits enrollment, housing, etc.
* Impart risk-reduction information and educate clients on building skills supporting safe sex behaviors and demonstrating strategies that can lower the risk of infection.
* Assist individuals in accessing additional services to support risk reduction that are not provided at Wyckoff Hospital, including substance abuse treatment, needle exchange, insurance enrollment assistance, etc
* Enter client data in a timely fashion in program-related documentation templates and databases and maintain accurate, complete, and up-to-date client charts.
* Enter and maintain accurate and complete outreach activity logs
* Assist in the development, updating, and procurement of promotional and educational materials for Prevention programs
* Provide community education and prevention resources to clients and staff of community agencies or groups serving key individuals at high risk for HIV and other sexually transmitted infections.
* Through individual or group sessions, provide instruction in behaviors and tools which decrease the risk of HIV infection and enhance relevant psychosocial skills to support health and wellbeing.
* Participates in regular supervision with assigned manager and other regular staff and program team meetings as required
* Attends necessary training and in-services
* Completes in timely fashion other job-related duties to meet program needs as assigned by the Program Manager.
* Demonstrates effective communication skills
* Very detail-orientated with the ability to manage multiple projects at a time.
* Highly proficient in MS Office Suite.
* Exceptional customer service skills and the ability to work with people from diverse backgrounds in a team environment.
* Strong organizational and communication skills, both verbal and written.
* Must be flexible and willing to work irregular hours, including evening and weekend events. Resourcefulness, creativity, and enthusiasm are highly desirable.
Qualifications:
Minimum formal education of an Associate’s Degree/ Two years of College + Two (2) years of related work experience OR four (4) years of related work experience (plus any additional required experience). At least one year of experience in HIV prevention and/or HIV counseling and testing working in Human Services, community organization, or program development is required. Excellent verbal and written communications skills with the capacity to motivate individuals and groups with diverse professional and cultural backgrounds.

Preferred qualifications: Bachelor’s degree in a related field. Knowledge of HIV and sexual health services; knowledge and experience with service delivery and community outreach to special populations such as LGBTQ, immigrants, and communities affected by substance abuse and mental health. Experience using new media technologies, including social media, smartphones, and apps. Training and active certification in HIV testing and counseling. Highly organized and self-motivated. * Bilingual English/Spanish preferred
