Advantage Rehab, part of the Corus Health family of companies, is currently seeking a qualified Physical Therapist to provide restorative care for the residents of Laguna Rainbow Elder Care Center, just outside of Albuquerque in Paraje on the Laguna Pueblo. Duties & Responsibilities:

• Evaluate and assess patient's level of function
• Developing an individual plan of care based on personal goals
• Implementing the treatment program through direct treatment of the patient
• Reassessing clinical signs and symptoms to determine effectiveness of program
• Instructing patient, family, and other clinical staff regarding participation in the treatment program

Requirements
Qualifications & Requirements:

• Degree in physical therapy from a university accredited by the American Physical Therapy Association
• License to Practice by the State of New Mexico
• Comprehensive knowledge of the principles, concepts, and methodology required of an physical therapist and its practice application in the treatment of patients in a rehab setting
• Excellent Communication and documentation skills
• Willingness to work cohesively with an interdisciplinary team
• Valid driver’s license and reliable, insured transportation
