How you will make an impact!

• Provide skilled, therapeutic services to clients, in their own homes according to a physician-approved plan of car
• Prepares and maintains accurate clinical documentation.
• Provides clear written or electronic documentation of all patient care provided and the patients progress toward their goals.
• Maintain strict infection control and safety practices.
• Participate in interdisciplinary team meetings, trainings, and assist teammates as needed.

PT Job Requirements:

• CPR Card (BLS)
• Must pass criminal background check, abuse background check and drug screening
• Must provide a valid Drivers License and current Auto Insurance
• Current PT licensed in the state of Wisconsin.

ADA Specifications:

• Able to constantly travel within geographic area serviced by office from assignment to assignment.
• Work is normally performed in a residence, aggregate living facility, school or occupational setting.
• Heavy physical activity that may require lifting, carrying, pushing or pulling over 50 lbs., and physically stretching, bending, stooping, twisting, standing, sitting, walking, reaching and climbing stairs.
