    Position Summary
      Medication reconciliation technician (MRT) is responsible for accurately collecting and entering patient’s current medications, allergies and medication sensitivities into the hospital's information system upon admission. MRT will primarily be working in the Emergency Room with frequent contact with patients, nurses and physicians. This position requires the technician to be flexible with scheduling needs and be able to perform other responsibilities as required by a Pharmacy Manager or Technician Supervisor.
    Required Minimum Knowledge, Skills, and Abilities (KSAs)
    • Education: N/A
    • License/Certifications: Currently licensed as a pharmacy technician in the State of Maine or currently eligible consistent with the current State of Maine Board of Pharmacy requirements. Current pharmacy technician certification (CPht) (or achievable within 6 months of the date of hire) received through examination administered by the Pharmacy Technician Certification Board (PTCB). National certification required.
    • Experience: Minimum of two years' hospital/retail pharmacy or medication management experience preferred.
    • Excellent communication skills with the ability to interview patients and/or their family members and relay medication discrepancies to clinical pharmacists.
    • Considerable knowledge of medication names (both trade and generic), medication uses, doses, and routes of administration, as well as over-the-counter medications and supplements preferred.
    • Ability to use medication resources to aid in medication reconciliation process, including, but not limited to, Health Info Net, Maine PMP, Lexicomp, UpToDate and retail pharmacies.
    • Recognize barriers to obtaining information (language, social, mental status, etc.) and actively problem solve solutions
    • Computer skills and knowledge of different programs such as QS-1, Epic, Excel, Outlook and Word is preferred.
