Job description

Responsibilities: Performs the discipline-specific functions of a registered pharmacist in providing Pharmacy services to patients and other health care professionals. Primarily focuses on the distributive functions including, but not limited to, order entry, order verification, problem resolution, and order clarification.

Performs pharmaceutical care activities including patient chart review, renal dosing adjustments, IV to PO conversions, target drug monitoring, monitoring of drug-food interactions, and patient counseling for targeted drugs and patient populations served.

Collaborates with members of the patient care team and assists in activities that promote quality patient care. Participates in assessing, planning, implementing, evaluating, and documenting the care of patients served by the patient care teams. Responsible for developing and monitoring cross trainable tasks performed by other team members.

Demonstrates population-specific competencies (e.g. age-specific, cultural sensitivity, patient-centered communication) in performance of role. Qualifications: Education
Bachelors Degree in Pharmacy or Doctor of Pharmacy Degree.

Licensure/Certification
Current Georgia Board of Pharmacy License.

Experience
Six months hospital Pharmacy experience preferred.

Knowledge, Skills and Abilities
Discipline-specific knowledge of growth and development as well as diseases and health care needs relevant to the ages of all patient populations served.

High level of interpersonal skills in order to effectively communicate with patients, visitors, team members, physicians, and other hospital personnel with whom daily interaction must take place.

Assessment, analytical, and synthesis skills sufficient to interpret and analyze patient health data, identify discipline-specific diagnoses, prioritize needs, and develop an individualized discipline-specific plan of care.

Evaluation skills adequate to effectively evaluate patient response to discipline-specific care and take appropriate action to promote desired outcomes.

Ability to think critically and prioritize multiple tasks and responsibilities as well as to perform efficiently and effectively under stress.

Problem solving skills sufficient to allow effective problem/conflict resolution.

Demonstrated traits of initiative and dependability required.

Reading comprehension and visual skills sufficient to see and interpret and verify physician orders and enter appropriate information into patient electronic medical record.

Hearing comprehension sufficient to communicate effectively in person or over the telephone.
