The Perinatal Social Worker provides comprehensive perinatal services in a community health setting. This position functions as a central person in assessing patient’s psychosocial, health education and nutritional needs, developing and coordinating patient care plans with the specialist team and maintaining service linkages with community agencies.
This is a full-time benefited position working 40 hours per week, typically Monday through Friday with periodic Saturday hours.

Responsibilities:
- Assures compliance with established procedures and quality standards of the Comprehensive Perinatal Services Program (CPSP).
- Performs initial orientation/intake of CPSP patients, conducting interviews to obtain necessary medical and obstetrical history, order labs and schedule appropriate appointment.
- Conducts initial psychosocial, health and nutrition assessment to identify high risk factors related to perinatal patients.
- Develops and provides re-assessments to assess appropriate interventions based on patients’ needs in various clinical areas including counseling, specialist, and other service referrals.
- Follow up with post-partum assessments and care plan.
- Provide support to patient with disability forms.
- Provide individual feeding support for newborn babies.
- Conducts support groups, parenting classes, presentations, and other support services necessary to improve/enhance program services as needed.
- Coordinates service delivery and performs administrative tasks to integrate Family Planning services with other aspects of patient care including medical, nursing, laboratory, client/patient accounts, and other areas as needed.
- Attends and participate in the clinic case conferences and staff meeting.
- Participate in County and State perinatal meetings as needed.
- Performs all duties in accordance with Service Excellence Standards of TVHC, Inc.
- Performs all duties in support of successful EHR/EPM implementation.
- Performs other related and administrate duties as assigned.
Qualifications:
- Proven ability to work effectively with individuals and groups of various ages, cultures, and backgrounds, especially in dealing with sensitive topics, providing services in a non-judgmental and ethical manner. Ability to provide all client services with a customer-service focus, exercising confidentiality, professionalism, and diplomacy.
- Must be sensitive to the needs of the community.
- Written communication skills in English adequate to draft reports, memos, business correspondence, etc., as needed or directed; basic written communication skills in Spanish to communicate to patients / patients' families, as needed, to provide them with necessary information.
- Ability to work effectively both independently and as part of a multidisciplinary team.
- Must be a self-starter and able to accomplish expected tasks under minimal supervision; Ability to multi-task and maintain composure under pressure.
- Familiarity or willingness to learn about CA State Comprehensive Perinatal Services and other related programs.
- Intermediate knowledge and experience working with computers, including word processing and data entry programs; spreadsheet experience a plus.
- Knowledge of or willingness to learn reporting laws for Domestic Violence and Child Abuse or neglect.
- Able to meet or exceed the Service Excellence Standards of TVHC, Inc.
- Must have ability to travel to designated TVHC locations.
Education and Experience:
- Minimum one year experience in maternal/women’s health and child health required.
- High School Diploma required.
- Bilingual in English/Spanish required.
- Medical Assistant Certificate preferred, not required.
- Bachelors or master’s degree in Social Work strongly preferred.
- Experience in providing social services and counseling to patients in a health care setting preferred.
- Experience using EPIC preferred but no required
- Prior experience working in a multi-disciplinary community services clinic highly preferred.
