Job Description:

• Work cooperatively with the Primary Therapist for designated clients to assist the client in implementing their Master Treatment Plan and support designated clients in transitioning across levels of care as called for by their Master Treatment Plan.
• Assist clients in developing problem-solving skills to assist in carrying out the requirements of everyday living, including assisting in securing safe and affordable housing, securing employment, making and keeping appointments for appropriate community services, arranging necessary transportation, etc.
• Assist designated clients in developing new activities and behaviors to support sustained recovery.
• Work with the nursing and clinical teams to develop and conduct appropriate support groups.
• Assure that all services provided are properly documented in an accurate and timely manner in accordance with the policies and procedures of Sacred Heart, the standards of CARF, and all applicable funder rules and regulations.
• Attend and participate in mandatory meetings such as Team Meetings and All-Staff Meetings. Complete all mandatory training at hire and annually when due. Follow all Sacred Heart Policies and Procedures, Corporate Compliance, and Code of Conduct.
• Perform other related duties as assigned by the Leader.

Qualifications:

• Must be in recovery from a substance use disorder and involved in a 12 step recovery program with a minimum of 2 years of sobriety.
• High School Graduate or GED.
• Credentialed as a Certified Peer Recovery Coach or have the ability to obtain.
• Good written and oral communication skills.
• Knowledge of 12 step recovery model.
• Demonstrated awareness of local community support groups and resources.
• Ability to work a flexible schedule.
• Must have good computer skills.
• No past violent crimes or weapons charges.
• Ability to adapt to change, motivate others, and provide empathic support.
