The Patient Engagement and Population Health (PEPH) Director works with the Director of Quality and Risk Management, , and Care Teams to direct quality and population health outcome-based programs for the organization. This position will also work closely with MPCA and other agencies on multiple projects and opportunities to increase access to care, promote patient engagement, and prepare KTCH for value-based care transformation. Travel to all KTCH locations will be required. The employee should have a valid Missouri driver’s license and state required motor vehicle insurance.

Position Responsibilities include:

I. Provides leadership to plan, organize, direct and coordinate Community-based services, care, case-management programs and activities, outreach, public education, and community-wide activities to improve the patient outcome, increase satisfaction with care delivery, and increased compliance closing the gap in health disparities and addressing social determinants of health.

II. Provide leadership and supervision to staff to maximize efficiency and achieve organizational goals.

III. Build and maintain favorable relationships with the organization’s leadership, peers, industry organizations, and community to improve teamwork and collaboration.

Minimum Qualifications:

· Ability to read, analyze, and interpret standard scientific and technical journals, financial reports, and legal documents.

· Ability to respond to common inquiries or complaints from customers, regulatory agencies, or business community members.

· Ability to effectively present information to top management, public groups, and the Board of Directors.

· Excellent Customer Service Skills

· Above average attendance
Experience in partnering with an executive team and have a high level of written and oral communication skills.

· .Strong communication skills that include diplomacy and straightforward instruction.

· Critical thinking skills and the ability to handle stressful situations.

· Must be able to lift up to 35 lbs. (without device assistance)

· Confidentiality of patient information is mandatory.

Education and Experience:

· Bachelor’s level RN or Bachelor’s degree in Business with 5 years of professional experience preferred

· Minimum 5 years of Supervisory/Management Experience

Job Type: Full-time

Pay: $39,044.47 - $131,873.78 per year
