Duties: What you will do:
The Palliative Care Program Coordinator supports palliative care patient management and productivity by managing intake, tracking, and scheduling of new palliative patient referrals, maintaining a comprehensive and accurate master roster, and performing confirmation calls; presents a caring and professional demeanor for all incoming calls, notifies agency staff members of pertinent messages; coordinates provision of patient documentation to primary care providers and other pertinent specialists in a timely manner; maintains patient privacy by adhering to HIPAA; supports agency staff members with regular and assigned projects under the direction of the Director and Assistant Director of Palliative Care.
Essential Job Functions/Responsibilities
Intake
1. Maintain accurate and comprehensive running log of new referrals for follow-up
2. Verify edocs file reflects palliative care order and palliative care consent, if consent signed by other than patient, verify edocs file reflects appropriate documentation of authority
3. Verify edocs file reflects appropriate history and physical
4. Verify intake and contact pages are complete to the fullest extent possible
5. Schedule initial consults to occur within two weeks of referral receipt
6. Assign NP per knowledge of territories and current caseloads
Communication
1. Conduct appointment confirmation calls
2. Fax visit notes on a weekly basis
3. Take general calls to office and palliative specific calls when available
4. Direct calls and messages appropriately to ensure timely attention to patient issues via active handoffs and follow-up confirmation
Requirements: What you need to have:
1. Some college, bachelor’s degree preferred
2. Previous similar experience in office and healthcare environments
3. Excellent verbal and written communication skills with a focus on outstanding customer service.
4. Ability to understand and manage phone system, fax, copies and other office equipment as required.
5. Excellent organizational skills
6. Proficient in computer systems and applications.
7. Knowledge of and commitment to customer service principles and practices.
8. Ability to work under pressure.
9. Professional personal presentation.
