Position Summary

Under the general guidance of the Clinic Manager, the Outreach Worker will work interpedently in the community with specific targeted populations. The Outreach Worker will work collaboratively with all healthcare professionals, social service agencies and various outreach programs to connect patients to resources to improve patient care and outcomes. Serves as an advocate for the health needs of CFHC patients and assists in supporting medical providers and the management team by creating community outreach programs and activities. Oversees outreach programs that are designed to promote, maintain, and improve the health of the patients and their family CFHC serves.

Essential Functions

  • Provide health education to community resident in the CFHC Service Area.
  • Conduct clinical services at health fairs and other community events.
  • HIV testing/counseling provided by certified or trained HIV tester/counselor based on center policy and protocol.
  • Promote CFHC as a primary health care option and introduce patients to our programs and payment methods.
  • Educate community residents how to adopt healthier lifestyles.
  • Disseminating information about available CFHC services and community resources.
  • Assist patients in making appointments to CFHC clinics and case management services


Education & Experience

  • MA (Medical Assistant) or Certified Nursing Assistant (CNA) Accreditation
  • Certified of trained HIV tester/counselor preferred but not required
  • CPR Certification will be required and maintained
  • The candidate must possess a valid driver’s license and Insurance
  • The candidate must be current with skills and training and remain updated in the health field
  • Participate in continuous quality improvement programs regarding community health
  • Candidate must be able to attend all assigned community health events, rotating Saturdays mandatory


Knowledge, Skills, Abilities

  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • Knowledge of community agencies and resources.
  • Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services.
  • Ability to problem solve and take initiative.
  • Ability to plan, implement, and evaluate individual client care plans.
  • Knowledge of transportation and other barriers to care that may be encountered by client.
