Job description

Modivcare is looking for an Exception Facility Liaison to partner with nursing homes, dialysis clinics, hospitals, and other medical providers that have clients with routinely scheduled, discharge or frequent trips to maximize scheduling coordination and information sharing. You will visit facilities to provide training and information about the non-emergency medical transportation system, and to learn about facility concerns. You will be a liaison for patients and case-managers alike throughout the state providing in-service training to facilities on how to use NEMT services. You will report into the Operations department.

You will...

  • Provide community-based outreach and education activity
  • Develop ongoing relationships with facilities through face- to- face meetings, phone calls and group presentations
  • Maintain positive relationships with facilities
  • Report to and work with Member Experience team and members of management when issues are uncovered
  • Provide training in person and by webinar to facilities on how to use NEMT services including web-portal trip request
  • Promote company programs with healthcare facilities and case workers
  • Travel to healthcare facilities
  • Provide post-meeting follow up/communication to healthcare facilities and Modivcare departments

  • High School Graduate or General Education Degree
  • 0-2 years previous work experience
  • Experience working with health plans and healthcare facilities
  • Experience as an outreach or provider relations representative
  • Valid Driver's License
  • Clean driving record
  • Can travel up to 80% of the time

Salary: Starting at $19.17/hour
