Options Counselor

Full-time 35 hours week

The Options Counselor's role is to assist individuals needing information about services and supports available to them in the community setting of their choice. The OC also connects individuals as needed to appropriate sources using a No Wrong Door (NWD) approach and assists the agency with ongoing resource development and outreach. The OC will also serve as a backup to the Information & Referral team as needed. Options Counseling is a core function of the Pioneer Valley Aging & Disabilities Resource Consortia (ADRC).
- Provides Options Counseling (a combination of person-centered needs assessments, referrals, preliminary care planning, and short-term follow-up) for individuals seeking support and residing in all settings, including community and residential environments. Assessments are conducted in-person (preferable), virtually, or telephonically and may occur in the home, hospital, or office.
- Assess the referred individuals' functional, health, and income status to determine eligibility and appropriateness for agency and community-wide services & supports.
- Provide short-term follow-up on referrals or information as determined appropriate.
- Actively network with community peers to find relevant resources and services for OC consumers and the agency.
- Work as a team member with staff from other GSSSI programs to coordinate care and resources for assigned consumers.
- Assist with community outreach as assigned.
- Prepare and maintain required Wellsky client records following policies and procedures written by EOEA and GSSSI.
- Commitment to obtain AIRS CR/S certification within one year of hire
Education and Experience
- Bachelor's Degree in a human services field or an Associate's Degree in a relevant field with two years of recent related work experience.
- Knowledge of long-term care and social service delivery systems, community resources, and the Aging Services Access Point network are desirable.
- Bilingual in Spanish, desired but not required.
