Coleman Health Services has been selected to be part of OhioRISE to work with Multisystem Involved Youth and their families! If you want to make a difference, we want you on our team! As a CME Care Coordinator, you will provide structured service planning and care coordination through high-fidelity wraparound as established by the national wraparound initiative. You will provide care coordination services to no more than ten intensive OhioRise youth. We will be serving Summit County!

What are we looking for from you?

  • Ability to conduct comprehensive assessments including the CANS assessment
  • Provide high fidelity wrap-around services
  • Convene and facilitate child and family team meetings
  • Develop and implement child and family care plans
  • Perform referrals and linkages to appropriate services and supports
  • Timely completion and submission of documentation
  • Complete training programs provided by CABHCOE

What Qualifications will you need to be successful as a CME Care Coordinator?

  • High school diploma or equivalent with 3 years’ experience (or 2 years’ experience with an associate’s/bachelor’s degree or 1 year of experience with a master’s degree) in children’s behavioral health, or a related public sector providing community-based services to children and youth and their families or caregivers
  • Ability to manage complex cases to work with children and families who are engaged in one or more child-serving systems
  • Able to effectively communicate with youth, caregivers, multi-disciplinary staff, and partner agencies
  • Have and maintain an excellent driving record

Preference given to the following candidates:

  • Master’s degree with LSW/LISW/LISWS, LPC/LPCC/LPCCS, MFT/IMFT/IMFTS
  • Has NPI and Ohio Medicaid numbers
