The Occupational Therapist is a qualified professional who directs, supervises, evaluates and provides Part B Medicare Occupational therapy services to patients in their homes or in Assistive Living Facility (ALF) accepted for care by the RVNAhealth.

Responsibilities:

  • Comprehensive evaluation of patient's level of function and participation in development of the total patient care plan in conjunction with the patient, family, and physician.
  • Work closely with other disciplines as needed, PT, SLP
  • Identification of patient and family needs, develop plan of care and carry out treatment as necessary in accordance to medicare guidelines
  • Participation in case conferences, family meetings, Assistive living facility (ALF) and patient coordination of care
  • Instruction of patient, family, and other agency health care personnel in the patient's treatment regime when indicated.
  • Provide proper Supervision and oversite of COTAs and ensuring proper communication and documentation
  • Identify and make appropriate referrals to next level of care as needed
  • Reports to the physician and other allied health professionals on the patient’s condition-obtains physician’s orders initially and every sixty (60) days thereafter.
  • Serves as a consultant to other health team personnel regarding occupational therapy for individual patients.
  • Maintains Continuing Educations courses as mandated by the State of Connecticut.
  • Work within EHR, WEBPT for all documentation
  • Understanding of Medicare PART B regulations

Qualifications:

  • Graduate with a baccalaureate degree of an accredited School of Occupational Therapy approved by the American Medical Association Council of Medical Education in collaboration with the American Occupational Therapy Association.
  • Current license to practice as an Occupational Therapist in Connecticut
  • At least two years Clinical practice as an Occupational Therapist and Home Health experience preferred.
  • Ability to adapt this knowledge and skill to a home setting.
  • Knowledge and understanding of the principles of home care and community health administration and practice in the home and ALF setting
  • Ability to teach and communicate goals to the home care team - in the home setting.

Pay: $80,000.00 - $102,000.00 per year
