- Evening Shift 3pm-11pm & Overnight Shift 11pm-7am
JOB SUMMARY:

Together, these facilities offer patients the best rehab and nursing care they can obtain after their hospital discharge.
- Utilize the nursing process in the care of designated residents. Document clinical observation and assessment on all assigned residents per regulations. Report findings of ongoing observations \ assessments to the designated Nursing Supervisor.
- Document effectiveness of plan of care. Demonstrate knowledge of basic test \ lab collection values.
- Collaborate with Interdisciplinary Team then revise/update the Nursing Care Plan.
- Administer medications and treatments in accordance with policy and procedure
- Respond appropriately to patient needs: Monitor, observe and document vital signs, treatments administered and patients’ responses. Demonstrate knowledge of equipment and procedures.
- Prepare resident assignments for Certified Nursing Assistants, giving consideration to the category of needs of the patient and competency of personnel. Communicate ongoing resident, family and facility plan \ concerns \ information to staff.
- Provide a Nursing Assistant assignment for each CNA on his/her team. Communicate to CNA pertinent resident care needs.
- Monitor and evaluate the flow of activity and delivery of patient care provided by the CNA’s (turning and positioning, meal times, bathing, etc.).
- Assure CNA documentation records (ADL’s BM, Meal intake, etc) are completed. Communicate to the Interdisciplinary Team members any pertinent resident findings
- Demonstrate accountability in professional practice development. Maintain CPR \ IV course up to date.
- Demonstrate accountability in practice to residents, family, colleagues, and the facility.
- Demonstrate an understanding of department priorities related to patient needs.
- When necessary, work a flexible schedule to provide continuity of care for all patients.
- Assist other staff members in completion of their duties.
- Orient new Nursing personnel as assigned.
- Must be a graduate of an approved school of nursing.
- Licensed and currently registered in New York State as Registered Nurse.
- Previous experience with geriatric patients is desirable.
- Must have a sincere desire to work with the elderly, chronically ill, and handicapped.
- Must have a thorough knowledge of medical, social, and psychological needs of residents.
- Knowledge of general nursing theory and practice.
- Pharmacological background in theory and supervised clinical experience with medications.
- Thorough knowledge of principles and methods involved in handling of sterile/clean materials and daily hygienic care of residents. Preventing spread of disease by proper disposal of infected material and cleaning of equipment.
- First aid assistance in emergencies, must be able to handle emergencies with calm and intelligent response.
- Must possess a spirit of cooperation and enthusiasm in order to create an atmosphere conducive to rehabilitation and growth.
- Must be able to communicate well verbally with residents, families and staff.
Working Conditions/Physical Requirements (ADA):
- Good general health and demonstrate emotional stability.
- Able to lift a minimum of 50 pounds.
- Able to cope with the mental and emotional stress of the position.
- Works in well-lighted and ventilated areas as well as throughout the nursing service area.
- Sits, stand, bends, lifts, and moves intermittently during working hours.
- Subject to hostile and emotionally upset residents, family members, personnel, and visitors.
- Subject to odors and exposure to infectious waste, diseases, and conditions throughout the workday.
