THE PROGRAM
SmartCare is a multifaceted innovative program that works in consultation with Primary Care facilities and rural communities that need additional mental health resources and support. This position will entail working in the various SmartCare program components, working as a team member with NP’s, psychiatrists, therapist and educators in providing telephone and televideo consultations with primary care providers and with patients and families to assist in treatment efforts and/or in facilitating referrals to relevant behavioral health services and other support services in relevant communities when indicated. Adults, children and families throughout San Diego County and Native American populations across the state are also served by the program.
THE POSITION
The Certified Nurse Practitioner (CNP) provides psychiatric consultations for primary care providers and guidance for clients and their families (pediatric patients). The CNP is a specialist in medical and mental health provider services who contributes their expertise as a member of a multidisciplinary team of therapists (LPHA) and educators. An example of tasks include:
· Provide consultative services via telephone or televideo in accordance with established procedures and protocols.
· Provide services independently under the supervision of a senior staff psychiatrist, with consultation available from a team of consulting psychiatrists.
· With the program’s current NP, maintain and answer provider phone line, with ability to recommend psychiatric interventions to providers in real-time on assigned days. Guarantee that the phone is available even during absences, scheduled and unscheduled, by eliciting coverage from available staff NP and psychiatrists.
· Ensure all medical documentation is completed in a timely manner and information is related to the primary care doctor as needed in whatever form they chose, to include faxing copies of our clinical reports.
· Using best clinical judgment, discuss clinical cases with psychiatrists who are scheduled to consult when a complicated case is presented from primary care.
· Serve as a resource to staff regarding behavioral health issues and medical issues as needed. Assist in promotional presentations to physicians and staff about programs.
· Serve as a resource to staff about medical, psychiatric questions they may have.
· Complete e-medical records and invoicing in accordance with contract procedures.
· Provide outreach and training, to pediatric practices to promote best practice models of integrative physical health and behavioral health care in the primary care setting.
· Assist in development of a weekly newsletter in conjunction with the staff psychiatrists and other staff about behavioral health topics that would be relevant to primary care providers.
REQUIREMENTS
· Graduation from an accredited nurse practitioner program, maintain a current license as an NP in California.
· Current DEA Number.
· Knowledge and experience in providing psychiatric care to people from diverse cultural backgrounds
· Ability to pass criminal/background clearance, TB, drug screen, COVID vaccine requirements
· Bilingual Spanish preferred, but not required
HOURS
10 hour hours per week as needed. Hours are flexible as available and to complement other team member schedules.
If more hours are desired, we have another program looking for up to 30 hours a week. The SPRUNG program consists of staff working with the homeless population in a resources center. These positions can be added together.
PAY
$55-$60 per hour based on experience
