Your Goal
The primary purpose of this position is to facilitate coordination of care and nursing care management to all Greater Family Health patients.
How You'll Help Patients
- Maintains patients’ electronic medical record in accordance with facility policies and procedures.
- Assists patients in obtaining prior authorization for medications and act as liaison between pharmacy and clinical health care provider to find which medications are covered by patient’s insurance.
- Assists patients with Prescription Assistance Programs (PAP).
- Assists patients in obtaining prior authorization for diagnostic studies and act as liaison between medical facilities and clinical health care provider to ensure correct diagnostic studies or procedures are performed.
- Serve as liaison between Health Center, other health facilities, organizations, and participate in community outreach.
- Assist with quality assurance and risk management.
- Reports to all scheduled work sites and shifts on time and fully prepared to engage in all job responsibilities; location of work sites and shifts will change at the discretion of the supervisor.
- Engage in all offered training, consultations and supervisions made available. Fully and professionally respond to and implement directives and advice offered by all supervisory and training personnel, as well as professionally offered feedback from coworkers.

What You'll Need:
- Graduate of an accredited registered nurse program, BSN degree preferable, but not mandatory.
- Licensure in State of Illinois
- Current BLS Certification
- Certification in specialty is preferred
- 3-5 years of experience in specialty
- Excellent writing skills
- Computer literate and has ability to utilize various software, word processing and spreadsheet programs.
