NAFI NY’s Foster Care program is seeking a part time Nurse (20 hours / week) to be a part of our NAFI Foster Care team of professionals. This program is a community-based Therapeutic Foster Boarding Home program designed for youth who have been traditionally treated in residential settings. Youth are placed in specialized therapeutic foster homes, supported by a team comprised of clinicians and case managers working in partnership with foster parents, birth families and involved community members to support the youth in reaching their full potential.
Core responsibilities include intake well screenings, assessments, medication education and medical appointment coordination. Working as a member of this team, the nurse would collaborate with team members and foster parents to provide and ensure overall quality healthcare coordination for each youth.
