Job description

Under the direct supervision of a Provider; Nurse Practitioner will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment including prescribing medications and evaluation of care. Nurse Practitioner will work collaboratively with the Nephrologist to meet the needs of the patients, supporting the delivery of policy and procedures.

ROLES AND RESPONSIBILITIES:

CLINICAL:

• Assess, diagnose, plan, implement and evaluate treatment/interventions for presenting patients.
• Collaborate with physicians in formulating treatment plans for health problems and follow-up.
• Performs evaluations and treatment procedures in emergency when necessary.
• Complete medical forms and documentation related to patient care.
• Schedule and track all referrals to other agencies and providers, with follow-up care provided.
• Prior authorizations for procedures and medications, with follow-up.
• Review patient labs and consult with provider.
• Prescribe and review medications.
• Provide on call coverage within the group.
• See patients independently.
• Other duties as assigned.

ADMINISTRATIVE:

• Establishes goals and objectives as assigned by Provider and/or Clinic Manager.
• Participate in professional development programs and maintain CEUs.
• Ensure all billing and coding of charges are properly documented in EHR.
• Provide proper charting and dictation to meet billing requirements.
• Adhere to clinic policies and procedures.
• Comply with HIPPA/Compliance regulations.

QUALIFICATIONS:

• Certified Nurse Practitioner
• Experience in Nephrology
• Excellent problem-solving skills and analytical thinking.
• Excellent customer service.
• Be a person of integrity and character, willing to embrace change, make a positive impact in the lives of patients and co-workers.
• Ability to work with staff members at all levels of the organization in a cooperative, team-oriented manner
• Ability to work in a very organized manner

Pay to be discussed DOE

Job Type: Full-time

Pay: $1.00 - $2.00 per year
