Starting rate for a minimum of 3 years experience $15.30/hr.

JOB SUMMARY

This position is a member of a multidisciplinary project team serving high cost, high needs patients who are the most frequent utilizers of health care service. The position brings an added dimension to the care continuum by connecting community members as a means of strengthening community support networks for patients and those who have a history of hospitalizations related to chronic and mental illness and/or co-occurring diagnoses. Using Timebanking as a community engagement and activation model based on reciprocity, this position focuses on assessing both needs and assets from patients to connect them with community members who are able to provide social support and thereby contribute to the overall health of the patient and the community. This position will engage community members in projects related to both patient and community needs.

ESSENTIAL FUNCTIONS

1. Builds Neighbor-to-Neighbor (N2N) group leads for N2N group meetings which are part of the continuum of care and are defined as networks of community members supporting health care providers and non-providers in caring for patients primarily within communities where patients live, work and play.

2. Promotes and monitors N2N transportation system within the N2N program.

3. Facilitate a collaborative process with patients/families/caregivers to define and create group projects to help patients build a plan and network of individuals/resources to help fulfill patient and community needs.

4. Actively recruit and provides information to all new members in a timely fashion during recruitment and outreach events.

5. Coordinate participation of new members into existing and new group activities according to pre-established schedule.
