Under the direct supervision of the Mental Health Assistant Director, the Wellness Coordinator guarantees that all regulatory and programmatic standards are upheld regarding supervision, storage, and handling of medication, moreover, taking the lead with all wellness initiatives. The Wellness Coordinator will have extensive knowledge in medication names, side effects, and its general purposes. The Wellness Coordinator will effectively train and oversee that staff members are providing all-encompassing services to the Individuals within the OMH Licensed programs- under minimal supervision. It is expected that the Wellness Coordinator works collaboratively with the OPWDD Wellness Coordinator to ensure universal practices, systemic uniformity, and cross-training.
Duties will include but are not limited to:

Attend medical/psychiatric appointments, as appropriate/assigned by Operations Director

  • Oversees complete Wellness regimen for the Individuals within the Community Residences, Treatment Apartment and Supportive Housing programs, including but not limited to- medical supplies, assistive devices, diets, etc.
  • Provide ongoing weekly education classes to residents regarding wellness activities including daily hygiene and grooming, symptoms management, dietary awareness, smoking cessation etc.
  • Meet with Behavior Intervention Specialists monthly to compare data while taking a wholistic approach
  • Coordinate the acquisition of annual/bi-annual updates- presenting them during admission, discharge, and internal transfer meetings. These records are to be validated within Precision Care prior to date of expiration. Collaboration with Management team is essential.
  • Teach bi monthly Medication Supervision course. Effectively oversee and conduct course compliance for all OMH employees
  • Implement new Wellness initiatives to attain better physical and mental health outcomes. Develop and maintain recordkeeping systems.
  • Facilitate a quarter communication meeting with partnership pharmacies, and/or Wellness collaborates. Send minutes to Operations Director(s) within 24 hours of event.
  • Submit weekend “wrap ups” which detail any follow up needed during the weekend.
  • For Disposals: **Facilitate proper medication disposal within 30 days of discontinuation or expiration according to the MedSafe procedure. Sharps containers should also be routinely monitored and disposed of according to general safe practices
  • Semi-annual Wellness Training OR as deemed appropriate: Ongoing outside-the-box initiatives to be presented agencywide. It is expected that the Wellness Coordinators work on this area as a collaborative effort. The Wellness Coordinator should be networked with appropriate sources to ensure RISE is ahead of brand-new efforts across Suffolk and Nassau counties.
  • Provides education and training to both staff and Individuals to ensure full understanding of medication- leading to self-management. Assess Restorative service goals to indorse advancement.
  • Complete functional assessments during the Utilization management reviews (UMR) meetings to capture the Individuals progress towards self-sufficiency
  • Effectively communicate with pharmacy, interdepartmentally, physicians and other clinical staff in both
  • Monitor pharmacy payments; Facilitate payments for medication(s) and medical supplies in conjunction with everyone; Explore cost-savings
  • Provide training to staff and Individuals on monitoring of Blood sugar and Blood pressures- including tracking mechanisms.
  • Revise forms, and procedures to ensure a smooth workflow. Submit to Operations Director for final approval prior to dissemination.
  • Present monthly report of medication compliance, errors, concerns, etc. at Staff meetings and directly to Program Administration
  • Alongside Operations Director, critically assess financials related to Wellness initiatives to ensure the most feasible, cost-reducing systems are in place.
  • Must be available to answer any questions and offer guidance during normal work hours
  • All other duties as assigned by Operations Director

CANDIDATE MUST HAVE: **

  • New York State Driver’s License, reliable transportation, and extensive knowledge of OMH medication protocols, side effects and possess the ability to educate and train on this information. Effective communication is required in both written and oral form. Candidate must have executive level leadership qualities, organization, time management and display the utmost professionalism and competencies.*
  • Preferred: pharmacy experience, clerical/administrative/medical records experience, 5+ years of customer service/engagement*

Education: HS Diploma with at least (3) years’ experience in an Human Service field.
